Support with GP's, Hospitals & Specialist Visits

Services by Mid-Cornwall Lifestyles

key elements

1. Individual Support

Formal contractual agreements have been in place for many years providing individual services for a variety of agencies:
Adult Care, Health and Wellbeing (ACHW)
Cornwall NHS Foundation Trust
Kernow Clinical Commissioning Group (KCCG)

2. Personal Budgets and Direct Payments (Formerly Independent Living Funds)

Using your personal budgets we can offer a service to suit your needs. Currently we provide a number of support packages ranging from 70 hours a week through 2 hours 7 days a week to 2 hours once a week. We also provide bespoke cover for when family main carers need their own break - from a weekend to a month.

3. Befriending and personal support

This area covers both the good things in life and assistance with sorting out a few necessary evils. It can include any or all of the following: shopping, sorting out bills, writing letters, organising holidays, walking dogs, attending concerts, providing transport and support with GP’s, hospital and specialist visits, attending court, liaising with police and negotiating with other agencies.

All in the name of choice and increased access to the things in life that most people take for granted, thus increasing independence and health and well-being.

4.Accessible Transport

We have a Volkswagen wheelchair accessible 8 seater van that is used for attendance at meetings for Trustees' voluntary committee duties, organisations, social outings or hospital visits. This vehicle is available and insured for members of the disabled community to borrow for their own visits or holidays.
We also have access to a jointly owned 16-seater wheelchair accessible Volkswagen minibus. Owned and maintained jointly by ourselves, Scope for St Austell and St Austell PHAB Club. This vehicle is mostly used for regular weekly meeting and mystery trips at weekends.

5. Advocacy

Over the last 10 years we have averaged working on 27 separate advocacy issues each year. The range of intervention has included meetings with the Police, Courts, Prison Service, Citizens Advice Bureau, Employers, Adult Social Care, Banks, Further Education, Benefits Agency, Funding Agencies, Solicitors, individual and group compensation insurance claims.
